Standard prostate biopsies suffer from high false negative rates and grade underestimation, creating an urgent need for improved tissue assessment. This study demonstrates the ability of label-free Fluorescence Lifetime Imaging (FLIm) for discriminating high-grade prostate cancer in ex vivo biopsy specimens immediately after collection. Using a single-fiber system for excitation delivery and fluorescence collection across four spectral bands, we achieved ROC-AUC of 0.88 for identifying Grade ≥4 cancer with 79.3%/85.0% sensitivity/specificity in specimens from 20 patients. These results establish FLIm as a promising modality for integration into next-generation optically-guided biopsy systems that provide real-time tissue feedback.

Julien Bec brings over 20 years of experience developing innovative solutions in mechanics, automation, and optics, with expertise spanning simulation, design, fabrication, project management, and medical device quality systems. Early R&D engineering roles with automotive companies established his systematic design methodologies for optimizing solutions within regulatory constraints. Since joining UC Davis Biomedical Engineering in 2009, Bec has applied this expertise to biomedical challenges, specializing in fiber optic components and fluorescence imaging systems for interventional cardiology and surgical oncology. As Director of Engineering for the "Optical Spectroscopy and Imaging for Disease Diagnosis" group since 2019, he oversees all FLIm device development, regulatory activities, and led the 20-patient pilot study in prostate biopsy presented here.
